Identity Verification for DUO Enrollment through Docusign

Before You Begin

You will need the following items:

  1. Your NetID (usually three letters and four numbers i.e. abc1234)
  2. A smartphone with a camera and stable internet connection
  3. A valid form of government-issued photo ID (passport, US State driver's license, US State ID card, or other ID listed in the verification step based off your country or region).

 

Step 1: Fill out the Form

  1. Visit Duo Multi-Factor Device Setup form
  2. In the NetID field, enter your Northwestern NetID
  3. In the Your name field, enter your First and Last names
    NOTE: Your first and last name MUST match your first and last name as listed on your legal ID or the verification check will fail.
  4. In the Your email address field, enter the email address to which you would like to receive the ID verification link.
  5. Complete the Verification Code reCAPTCHA
  6. Click the Submit button

Step 2: Complete the Docusign ID Verification

Please note, it may take up to one business day to receive the verification email. You will need a smartphone with a camera to complete the identity verification portion. 
  1. Go to your email inbox you listed in the Duo Multi-Factor Device Setup form and look for an email titled "Complete with Docusign: Northwestern University DUO Enrollment Request" 
  2. Click the Review Document link in the email to open verification web page
  3. On the Let's verify your identity page, click the I Agree button
  4. Follow the steps to select your Country or Region and select from the eligible types of ID 
  5. Continue the steps listed on the screen to verify your identity by uploading a picture of your ID and to taking a video of your face for comparison.
     
    NOTE: The steps will be slightly different if you are starting the process from your computer or a smartphone. 
  6. Once the ID verification process is completed, you will taken to the Docusign document
  7. Enter you NetID, Cell Phone Number to be used for Duo, and Email Address where you will receive your Duo activation instructions
  8. Click the Sign button to electronically sign it
  9. In the top-right corner, click the Finish button to complete the process

 

Step 3: Duo Account Activation

With your identity verified, IT support staff will enroll your account in our multi-factor authentication system, Duo. Keep an eye out in your email for follow up information and instructions on activating your account.
