Welcome to Northwestern! This form will help you complete the identity verification process required to register a device to use with Duo Multi-Factor authentication at Northwestern.
Once you fill out this form, we will send you an email from Docusign that will begin the identity verification process.
The email will walk you through the steps to verify your identity and will require:
- An ID from this Docusign list of elligible IDs.
- A phone with a camera to complete the verification process.
After you complete the ID verification process, we will send you a personalized link to register your device with Duo.
This process helps us secure any financial and personal data that you’ve shared or will share with us.
PLEASE NOTE: Northwestern University IT uses DocuSign ID Verification with Liveness to verify your identity. Please review this
