Microsoft Forms

Microsoft Forms is a simple, lightweight app that lets the user easily create custom surveys, questionnaires or feedback. Built-in analytics are available to then evaluate responses in real-time. Form data can also be easily exported to Excel for additional analysis. It is one of the Microsoft 365 applications offered at Northwestern.

Forms is designed for seamless integration across Microsoft 365.  Users can use Microsoft Teams to create Forms. Responses can be sent to SharePoint lists and form submittals can be used as a trigger in automated workflows using Power Automate.

How to Request Service

1. Go to https://office365.northwestern.edu. You will then be directed to the Northwestern Online Passport page for authentication.
2. Log in with your NetID and password.
3. Click on Forms in your list of Apps.
