CONDUITS Online Frequently Asked Questions

Prior to being able to access the CONDUITS Online form you must request access, note that not everyone has access, by submitting a ticket thru the Service Management Portal.  Click here to determine the existing Departmental Work Order Contacts.
Once access is granted, the form is NetID authenticated; meaning you use your NetID/NetID password to log in.

Yes, attachments can be included with the initial submittal thru the CONDUITS V 2.0 and greater Online form.
However, there is a 4MB limitation total for all combined attachments. Additional attachments can be added to the ticket thru the Service Management Portal.

• Navigate to the Service Management Portal -> Home -> Quick Actions -> View Your Tickets
• Locate/Preview your request by clicking on the 'Title"
• Drag and Drop attachments are limited to 4MB per attachment; while clicking the + symbol, selecting "from your computer" will allow attachments up to 50MB

Once you receive the system notification including the ticket number, you may update your request by responding to the email or thru the Service Management Portal.

• Navigate to the Service Management Portal -> Home -> Quick Actions -> View Your Tickets
• Locate/Preview your request by clicking on the 'Title"
• Click on the 'Comment' command button; the responsible resource will be automatically notified

However, dependent on the specifics of the CONDUITS Online Request, if the update is not received in a timely manner the work may have already been scheduled/completed.

Yes, but be advised that NM network activation (new install or update) requests will be routed to FSM IT for approval before the install or update process can proceed; you must be affiliated with NM for your order to be approved. This may delay completion of your request.

Deactivating a port and activating another port that is terminated in the same closet within the same building will not incur the full price for a port activation.

• Ports are terminated in the same closet if the first three characters of their labels are the same

Not Terminated Ports

• Ports are NOT terminated in the same closet if the first three characters of their labels are NOT the same

Submit a Request in CONDUITS

1. Go to The CONDUITS Online Login Page 
2. Click CONDUITS Online Order form
3. On the CONDUITS Order form, utilize the steps below:

Request Information

• Service = Select "Jack" from the drop-down menu
• Request = Select "Activate or deactivate a jack" from the drop down menu
• Request Details = Select "Jack(s) exist(s)" from the drop-down menu

Service Table

• Select the jack that is to be activated or deactivated in the Service Table.

Request Summary

The selected jacks will appear in the Request Summary.

• Select "Standard Activation" next to the jack that is to be activated
  • Enter Subnet, if known

Submit Request

• Department = Select "Department" from the drop-down menu
• Check the box next to the question if you would like this order to be flagged as a "Rush order"
  • If the box is checked, you will be provided with an option from the drop down menu to select the "Rush Order Rate"
• Check the box next to the question if this request was submitted on behalf of someone else.
  • If the box is checked, you are required to enter the Requester's contact information below
• Click = "Submit Request"

Request Information

1. Service = Select "Voice" from the drop-down menu
2. Request = Select "Request help moving phone service" from the drop-down menu

Service Table

1. City = Select "City" from the drop-down menu
2. Building = Select "the Building location" from the drop down menu 
3. Floor = Select "Floor" from the drop down menu

After this, the list of jack locations will be below.

Please select the jack(s) by checking the box next to the location.

Attachments

Attachments are used to provide additional information related to your request (i.e., pictures of jacks, floorplans, etc.). It is not used as a replacement for generating requests. See illustration below:

Request Summary

For this section, you will see a summary of the requested phone number, the jack selected of where the phone service is moving to, and a location where the field technician should meet you prior to moving phone. At this point, you have the option to edit your selection by removing and selecting another jack location. See image below.

Submit Request

• Check the box next to the question if you would like this order to be flagged as a "Rush Order"
  • If the box is checked, you will be provided with an option from the drop-down menu to select the "Rush Order Rates"
• Check the box next to the question if this request was submitted on behalf of someone else.
  • If the box is checked, you are required to enter the Requester's contact information below
• Click = "Submit Request"

• Add Phone # - This is optional if there is an existing phone number that will be used for the install
  • Leave this field blank if a new phone number is required
• And install – Select phone type from drop down menu

Request Information

Install Voice for Software Client Form

In this section, you will include the information required to build out the software only phone line. You can also click on the “Knowledge Base Article” for additional information about the recommended headsets to be used with the software client as well.

The labeling standard for faceplates/jacks is as follows: (using 03A-03062 as an example)

The following picture shows the actual faceplate of the above example. This faceplate has two jacks, A and B. B is open and active. A, having a black plug, is not currently active but is available to activate with an order. A beige plug indicates there is no cabling behind the jack and it cannot be activated without a wirepull.
