Individuals can physically move their Cisco phone from one location to another without the assistance from a Northwestern IT technician. However, the jacks where the phones are moving to must already be active.

Here are step by step instructions for relocating your phone.
- Determine that the jack you are moving to is already active and programmed on a subnet. This could be performed by doing one of the following:
- See if there’s a phone set already plugged into the desire jack.
- Go to https://services.northwestern.edu/TDClient/30/Portal/Home/ to create a ticket request. Be sure to include the Building name/address, Room number and jack number. Also, provide the phone number of the service moving, so that NUIT can validate the existing subnet.
- Please be sure to connect the phone set to your PC/lap as illustrated below.

Request Information
- Service = Select "Voice" from the drop-down menu
- Request = Select "Request help moving phone service" from the drop-down menu

Service Table
- City = Select "City" from the drop-down menu
- Building = Select "the Building location" from the drop down menu
- Floor = Select "Floor" from the drop down menu
After this, the list of jack locations will be below.
Please select the jack(s) by checking the box next to the location.
Attachments
Attachments are used to provide additional information related to your request (i.e., pictures of jacks, floorplans, etc.). It is not used as a replacement for generating requests. See illustration below:
Request Summary
For this section, you will see a summary of the requested phone number, the jack selected of where the phone service is moving to, and a location where the field technician should meet you prior to moving phone. At this point, you have the option to edit your selection by removing and selecting another jack location. See image below.
Submit Request
- Department = Select "Department" from the drop-down menu
- Check the box next to the question if you would like this order to be flagged as a "Rush Order"
- If the box is checked, you will be provided with an option from the drop-down menu to select the "Rush Order Rates"
- Check the box next to the question if this request was submitted on behalf of someone else.
- If the box is checked, you are required to enter the Requester's contact information below
- Click = "Submit Request"
